A point-of-care troponin study is a pivotal clinical investigation in all but name. Patients presenting to emergency departments with suspected acute coronary syndrome are enrolled, serial samples are drawn at protocol-defined times alongside standard care, and a blinded committee adjudicates whether each patient had a myocardial infarction using the Universal Definition. Device results are evaluated against both the laboratory troponin assay and the adjudicated diagnosis, and sample size is set by the number of adjudicated infarctions, not by the number of patients.

High-sensitivity troponin raises the analytical bar as well. Sex-specific 99th percentile upper reference limits must be established in a well-characterized healthy population, and imprecision at those limits must meet the accepted definition of a high-sensitivity assay. Point-of-care platforms using whole blood face the added question of agreement between whole blood and plasma at the very low concentrations where rule-out decisions are made.
